Children and Family Services has identified a small subset of children in foster care who, by meeting specific criteria, have been determined to have complex needs and may benefit from enhanced and individualized care coordination. Case Management Field Service Specialist Unit will manage these referrals and work with the custodial agency to achieve permanency for these children.
The goal of this service is to collaborate with and mentor the custodial case worker through the following:

Family engagement

Enhanced relative and fictive kin search efforts

Referrals for services and placement

Assistance with documentation
-
Remaining open-minded, solution-focused, and knowledgeable of service opportunities

Coordinate training and support for placement providers

Facilitate meetings with assigned action steps for all team participants

Safety Planning and Crisis Management
During this timeframe, action steps for continuity of care by all partners will be put in place. With the additional focus in each of these areas, the goal for the conclusion of comprehensive case coordination services is reached when there has been 6 months of placement stability in a family home with an identified and realistic plan for permanency. The Comprehensive Case Coordinator (CCC) will remain peripherally involved for aftercare oversight as the Case Management Field Service Specialist (CM FSS) throughout the life of the case.
